please fill in this form (PDF file) or (DOC file) and send with your light.
The more detail you can give me the better.

If you currently have an existing bike light that just can't cut it any more, like the bulb is blown or the ballast doesn't work, maybe your battery doesn't last as long as it used to.

It doesn't even have to be HID!

Switches are tired or not working.

Cables are broken (except curly cords).

I may be able to repair your light for you.

In some cases it just isn't feasible to repair.
